Online Registration – FinalForms
Bellingham Public Schools has partnered with FinalForms for online registration. Families, along with your student athlete, need to complete/update an athletic registration each year. In addition to the forms, the student will need a current sports physical on file. Students can register and clear through FinalForms at any time, however, in order to play they must be cleared the day prior to the first practice.
The emails listed in Skyward are used for communicating in FinalForms. If your email has changed, please contact the Registrar and Athletic Administrative Assistant. This Parent Playbook will also help you with step-by-step directions. If you require any support during the process, scroll to the page bottom and click “Use Support.” Families, along with your student athlete or activity member, complete a registration form each year. A benefit of the online registration is that your information is saved making the process easier when registering for athletics next school year. This also allows the athletics and activities office to verify that all forms are turned in prior to the first practice ensuring your student will be able to participate. Computers are available for families with limited internet access and you may call the athletics and activities office at (360) 676-6470 x7634 for support while filling out the online registration.
Important:
- After completing the online registration forms, your student will need to be cleared for each sports season through their school’s activities office before they can participate in any sport. You may review your data at any time to verify it is current, and you will sign your forms once per year and after any updates you make.
- In addition to the forms, your student will need a current sports physical on file. You can find the required forms on the WIAA forms website under "Health and Wellness Forms." The sports physical cannot expire during the sport season and is required for the student to participate in athletics. You can get a sports physical from your regular health care provider.
- Physicals can be uploaded to FinalForms, scanned and emailed to Pamela.Behee@bellinghamschools.org, or mailed to Squalicum High School.
- All eligibility forms can now be found in the WIAA Student Eligibility Center. These include forms required if athletes are participating in Running Start, Home-Based/Home Schooled, Alternative Education or International Exchange. Please contact Marisa Cantu for assistance with filling out the WIAA Running Start form.
No ASB Card payment is required.
High School Athletics Fee - 2026-27
In 2026-27 we are implementing fees for WIAA high school sports. Funds collected help cover growing costs, including transportation and officials.
Here's what families and athletes need to know:
- High School sports: this athletic fee is required for high school student athletes.
- WIAA sports: this includes cross country, football, flag football, soccer, softball, swim/dive, volleyball, basketball, gymnastics, wrestling, baseball, golf, tennis, and track & field.
- Fee amount: $100 per student per sport; the third sport is free.
- How to pay: Visit our online payments webpage to pay athletic fees online, or pay in person at the ASB bookkeeper at our school.
- Payments must be received prior to first competition/contest.
- Families who qualify for free and reduced meals do not have to pay; please fill out the fee waiver form in FinalForms..
- If financial assistance is needed, contact your school's ASB office. A student can still participate in sports even if a family cannot pay.
- Refunds: We will provide refunds under the following conditions. Refunds must be requested through the athletics and activities coordinator prior to the end of the sports season.
- Athlete does not make the team for cut sports (girls basketball, boys basketball, and volleyball)
- Athlete quits due to illness or injury prior to first competition/contest.
- If you have questions about the athletic fee, please contact the athletics and activities coordinator or the ASB bookkeeper.

Family Pass and Punch Card
Want to save on attendance fees for sporting events with an entry fee? See below
- The Family Pass $20: Family passes are available in the ASB office to families of athletes (football, volleyball, basketball, and/or wrestling), cheer, dance and band members attending events. The pass includes admission for two parents/guardians and siblings 18 and younger to attend home regular season games and matches at BHS, SHS and SQHS. Fall and winter seasons are sold separately; no other games (e.g. post-season) and no extended family sales are available.
More information:- Example: My SQHS student plays football. With the family pass, my athlete's siblings (18 and younger), my spouse, and I have entrance into home football games at BHS and away games played at BHS or SHS.
- If my SQHS student also plays basketball in the winter, I will need to purchase another family pass for the basketball season.
- These passes are for regular season games, not postseason games such as districts or state.
- Family Passes are ONLY sold at the ASB office. Students can purchase for their families.
- The Punch Card $40: The punch card costs $40 and provides 10 entries into regular season games, making the price of entry only $4. This pass is the most flexible option we have. Anyone can purchase, and anyone can use. You can purchase punch cards at the ticket gates. Passholders can attend any regular season games, and the pass lasts all year. If you use your ten “punches”, you can easily purchase a new one at the ticket gate.
