Mission Statement
The Squalicum High School athletic department will create and foster an environment that provides opportunities for student-athletes to enrich their high school experience through participation on athletic teams, which are competitive at the conference and state level.  We are dedicated to providing opportunities, which will enhance the intellectual, physical, social, moral and cultural development of the whole person, while conducting all activities with honesty and integrity in accordance with the principles of good sportsmanship and ethical conduct.

Once you have completed the FinalForms athletic registration, students complete the clearance process by purchasing an ASB card, and confirming their choice of sport with the Athletic Administrative Assistant in the main office. Students need a sports physical exam on file–a current, valid physical cannot expire during the current season.  Physicals are valid for TWO years from the date of the physical.





Online Registration — Parent and student portions of Registration on FinalForms must be completed before a student can be cleared to practice.  Please select the sport(s) the athlete wishes to participate in during the year.

* Current Physical — needs to be on file in Athletic Office–physicals are valid for 2 years from date of examination–sports physical exam Physicals can be uploaded to FinalForms, scanned and emailed to, or mailed to Squalicum High School.

* ASB Card – $40 (full-pay athletes) or $20 (Free or Reduced Lunch athletes). There is no athletic participation fee but an ASB card is necessary to participate.  If you need any assistance, please contact the ASB Office at

* Fines or fees — Current policy is that all fines and fees must be paid before an athlete is eligible to clear.  However, if you need any assistance or have questions about your account, please contact the ASB Office at

*Clear through the Athletic Office.

  • After completing the FinalForms Registration, the parent/student must contact the Squalicum Athletic Office to be CLEARED for their individual sport.
  • The Athletic Secretary will verify that the FinalForms Registration is complete, there are no outstanding fines, and that the student has a current sports physical exam on file.  


  • After completing the online registration forms, your student will need to be cleared for each sports season at their school’s activities office before they can participate in any sport.
